Meet Dr. Shiv Kumar Goel, MD

Hi, Dr Goel, let’s start with a bit about your interesting background.
Hi Dr Yildiz, thank you for the opportunity. I’d be glad to share a few key points regarding my background.
I was born in New Delhi, India, into a family of nine with limited resources. That shaped my resilience, my drive, and my deep empathy for people who feel unseen by the system.
My father was a mathematician who devoted his entire life to caring for our large family while neglecting his own health. He battled major heart disease, diabetes, hypertension, and kidney problems, and without any medical professional in our family, no one truly understood what was happening to him or how to help.
I trained in anesthesia and critical care in India, then came to America to pursue internal medicine at Mount Sinai School of Medicine in New York. I spent years as a hospitalist, became a medical director at Methodist Hospital in San Antonio, and taught residents as an Assistant Professor at Texas Tech.
Today, I am a board-certified Internal Medicine physician and founder of Prime Vitality Wellness in San Antonio, Texas, where I blend evidence-based medicine with functional, lifestyle, and aesthetic approaches.

Why did you choose this amazing profession?
Three defining moments led me to medicine.
First, I was always a curious student who carried “why” and “how” on his mind. Biology and science captivated me from childhood. I wanted to understand what made living systems work.
Second, watching my father’s health deteriorate shaped me profoundly. He gave everything to our family and neglected himself. His struggle with heart disease, diabetes, hypertension, and kidney failure, without anyone in our home who could truly understand what was happening, planted a seed in me. I wanted to be the person who could understand, who could help.
Third, in eighth grade, I developed a severe allergic eye disease that lasted months. My mother took me from doctor to doctor. During one visit, I watched patients wait in long lines to see a physician who had barely five minutes for each of them, with no explanation, no connection, just a prescription and a dismissal.
In that moment, something shifted. I knew I wanted to practice medicine differently, to actually see patients, listen to them, and help them understand their own bodies.
What inspired Prime Vitality?
After years of practicing conventional medicine, I saw the same fractured system everywhere, whether in India or the United States. Patients were receiving band-aids, not solutions. No one was addressing the root causes or helping people understand why they were sick in the first place.
I decided to create a platform where we do not just treat symptoms but help people understand the real reasons behind their health issues and, by addressing those root causes, not only prevent disease but optimize health to a level where we can truly experience and enjoy life at its fullest.
I studied functional and integrative medicine through the American Academy of Anti-Aging Medicine (A4M) and aesthetic medicine through AAOPM. I pursued advanced training in peptide therapy and regenerative medicine, bioidentical hormone replacement, longevity protocols (Longevity 101 and 201 certifications), and the latest developments in gene therapy and precision medicine.
This comprehensive approach allows me to bridge the gap between conventional medicine and the future of personalized, preventive care.
Prime Vitality was born from this vision, a place where cutting-edge science meets ancient wisdom, where patients are partners in their healing, and where we treat the whole person, not just the disease.

What do health and well-being mean to you?
Health and well-being mean wholeness.
True wellness is not the absence of disease but the presence of vitality at every level — physical, mental, emotional, and spiritual.
My mission is to blend cutting-edge medical expertise with Eastern holistic ancient wisdom, motivational insights, and the power of AI integration through what I call “Time Vitality,” using temporal biomarkers, circadian rhythms, and seasonal awareness to help people reclaim their energy and longevity.
What are your hobbies, or what excites or entertains you?
Daily meditation and breathwork are non-negotiable.
I have completed Vipassana silent retreats, Art of Living courses, and Sadhguru’s Inner Engineering. Beyond that, I am fascinated by astronomy, space weather, and how cosmic rhythms influence our biology.
I recently competed in men’s physique bodybuilding, a journey that taught me as much about discipline and self-belief as any spiritual practice. I also love writing poetry and exploring consciousness studies.
What are the top three books that affected your life?
- The Power of Now by Eckhart Tolle — It helped me understand presence and the grip of the mind’s narratives.
- Autobiography of a Yogi by Paramahansa Yogananda — A bridge between Eastern mysticism and Western science.
- Man’s Search for Meaning by Viktor Frankl — A reminder that meaning can be found even in suffering.
Now, let’s talk about your writing a bit. Firstly, why do you write on Medium?
Medium provides a unique space where I can blend personal narrative, clinical insight, and philosophical reflection.
Medium allows me to reach readers who are searching for more than surface-level health advice, those who want to understand the why behind the what.
I write to process my own transformation and to leave breadcrumbs for others walking similar paths.
Do you write on other platforms like Substack?
Yes, I run two Substack publications: Time Vitality Lab and Healing the Split.
Substack offers a more intimate, long-form relationship with readers. I find it ideal for deeper explorations of circadian biology, AI in healthcare, and the integration of ancient wisdom with modern science.
I also publish on KevinMD, Op-Med, Doximity, Elephant Journal, and contribute to Texas Medicine.
What are your values as a writer?
- Authenticity: I write from lived experience, not borrowed wisdom.
- Integration: I bridge Eastern and Western, ancient and modern, science and soul.
- Service: Every piece should leave the reader with something they can use.
- Vulnerability: The stories that matter most are the ones we are most afraid to tell.
How do you connect with your readers?
Through personal narrative. I share my struggles, such as burnout, spiritual crises, and the dissonance between being a healer and feeling broken, because I believe authenticity creates resonance.
I also engage actively on LinkedIn and Medium, responding to comments and building relationships beyond the page.

What are the success factors for you as a writer?
- Consistency: Showing up daily, even when the words do not flow.

- Integration: Letting clinical knowledge, spiritual insight, and personal narrative inform each other.
What do you recommend to new writers?
Write from your scars, not your wounds. Find your authentic voice before worrying about algorithms. Read widely, but write narrowly — go deep into what you truly know. And remember: the readers who need your words will find them.
What are your plans as a writer or professional?
I am completing my book Healing the Split: Reconnecting Mind, Body, and Spirit in Modern Medicine. I am also building Time Vitality, an AI-driven holistic healthcare ecosystem that integrates circadian biology, temporal biomarkers, and personalized wellness guidance.
Through clinical practice, writing, and thought leadership, I aim to inspire others toward their best selves, physically, mentally, and spiritually.
